Only a mere 21% were confident they could meaningfully intervene within thirty minutes. This isn’t just a minor oversight; it’s a fundamental failure in operational governance. It means that a corrupted or malfunctioning AI agent could continue making autonomous decisions, potentially causing financial loss, reputational harm, or even physical damage, entirely unchecked.

The core issue is one of control. If a business cannot quickly halt an AI system, explain its behavior, or identify who is accountable, then the business is not in control of that system. When a serious AI incident does occur, only 42% of respondents expressed confidence in their organization’s ability to analyze and clarify what happened. The Blame Game Nobody is Playing

A full 20% of professionals simply did not know. Only 38% pointed to the Board or an Executive.
